New Patient Registration

We welcome applications from all those living in the practice area (3 mile radius, south of Staines Bridge) and we aim to give top quality care to you, whatever your age, gender, ethnicity or lifestyle.

If you would like to register with our practice, please bring your medical card to the surgery if possible. Also bring some photo ID and proof of address.

You will be registered with the practice and can see any doctor of your choice.

We also like to invite you for a New Patient Health Check. This takes around 20 minutes and is an opportunity for a general health check with lifestyle advice.

If you do not have your medical card, please fill in our permanent patient registration form or new-born baby registration form, both available from reception, or you may download a copy below:
